With support from the Robert Wood Johnson Foundation, HWS studied 171 innovative practices across the country. Many of the final twenty-four innovative care delivery models share common elements like elevating the role of nurses; deploying interdisciplinary teams; bridging the continuum of care by extending the focus beyond the primary care setting; targeting high-users of health care such as the elderly; and actively involving patients and their families in care planning and delivery. Most importantly, all the models chosen have the potential of being easily replicated in a wide variety of healthcare settings.
