HWS launches a new website

Innovative Care Models provides detailed information on 24 promising quality improvement initiatives that are changing the way health services are delivered in communities across the country.

With support from the Robert Wood Johnson Foundation, HWS studied 171 innovative practices across the country. Many of the final twenty-four innovative care delivery models share common elements like elevating the role of nurses; deploying interdisciplinary teams; bridging the continuum of care by extending the focus beyond the primary care setting; targeting high-users of health care such as the elderly; and actively involving patients and their families in care planning and delivery. Most importantly, all the models chosen have the potential of being easily replicated in a wide variety of healthcare settings.
